Forms for Faculty and Staff

The Cultural Approval Request form for Trans4mations credit must be completed by a faculty or staff member. Students cannot request Trans4mations credit. If you are advising a student group, please submit the form on the group’s behalf. Cultural events must be approved before the event takes place. Cultural events may include art shows, performing arts, lectures, civic engagement, popular culture and workshops that engage students in an activity that addresses some aspect of culture as well as transitional programming such as career development events. YEAR 2 SERVICE FORMS

List of Pre-Approved Service Sites

Coker College has long partnered with organizations to provide students with the opportunity to serve throughout the community during specific service days, requested events or whenever they are available to volunteer. The sites on this list are already approved for Trans4mations credit. However, if there is an organization that you would like to volunteer at and it is not on the list, please contact Darlene Small at dsmall@coker.edu or 843-383-8039. All Trans4mations service should be approved before completing hours.

Pre-Approved Service Opportunities

Forms for Students

Students completing service projects for Trans4mations credit must document their hours by using one of the designated service validation forms. Individual students who are participating in a one- time opportunity should use the individual service form. If a student will be completing multiple hours at a single site, the student should use the multiple hours form and ask the site supervisor to sign it when all of the service hours are complete. The service validation form for student groups includes a sign in sheet to list all participants’ names. Student ID number(s) should be included on all forms to receive Trans4mations credit.

Forms for Faculty and Staff

Faculty and staff led service projects are also eligible for Trans4mations credit. Please submit the service approval request form prior to completing the service hours. All student participants should sign in and give their Coker ID number to receive Trans4mations credit.

Forms for the Community

Coker College welcomes the opportunity to provide service to our community. Agencies that are in need of assistance should complete the service request form at least two weeks in advance of your service project. The earlier the notice the more time we have to contact our students about assisting you with your service project or event. Also, Coker students can receive credit for service hours and may have a form that they will ask you to sign. Please help them to document their service hours.
